Payment Policy

As a courtesy to you, we will file claims to your insurance company. We ask that you bring complete insurance information regarding your policy with you, including your insurance card. Any fee, co-payments, or deductibles are due at the time of service.

You will receive monthly statements until your account is completely paid. These statements include money due from the insurance company and any balance you owe. If you think there is a problem with your coverage or if your insurance has not paid in 90 days and you are receiving the bill, please call your insurance company.

If you have any questions about your statement, please call our office at 629-5343. Look in the lower right hand corner for any payments you owe. As the parent, you are financially responsible for any costs the insurance does not cover.
